Make outpatient visits to clients in designated territories.
Perform mobility, ROM, balance, and safety assessments.
Develop and implement individualized care plans (manual therapy, gait, balance).
Assess and revise PT care plans; coordinate with nursing staff.
Educate clients and families on rehabilitative care and independence.
Document observations and interventions in EMR on a tablet.

🎯 Requirements

PA Physical Therapist license.
At least 1 year PT experience.
Graduation from APTA- or AMA-accredited program.
Able to work independently and manage time.
Strong interpersonal, organizational, problem-solving skills.
Proficient with computers; EMR experience preferred.
