Outpatient Physical Therapist, PT - Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility and ROM assessments, transfers, balance, strength, endurance.
• Develop and implement individualized PT care plans with manual therapy, gait, and balance training.
• Continuously assess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care for independence.
• Document care via EMR with accurate observations.

🎯 Requirements

• PA Physical Therapist license.
• Minimum of 1 year PT experience.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
