Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated territories.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Continuously assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• PA Physical Therapist license (PT).
• Minimum of one year of PT experience.
• Graduation from an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
