Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, strength, endurance).
• Develop and implement individualized care plans (manual therapy, gait/balance training).
• Continuously assess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions using a tablet EMR system.

🎯 Requirements

• Current PA PT license.
• At least 1 year of PT experience.
• Graduation from an APTA/AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
