Perform outpatient therapy evaluations to develop care plans for adults in senior living or group homes.
Use evidence-based approaches to maximize functional independence.
Coordinate with home health therapy partners to transition care to outpatient.
Educate clients, families, and community caregivers on care plans for team-based care.

🎯 Requirements

Current North Carolina OT license.
Minimum of one year OT experience.
Graduation from an OT program accredited by AMA/AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
