Outpatient Occupational Therapist, OT - Home Health

📋 Description

• Perform outpatient therapy evaluations to plan care for patients in senior living communities or group homes (adults with intellectual disabilities).
• Maintain licensure level and use evidence-based care to maximize client independence.
• Collaborate with home health therapy partners to transition care to outpatient to meet client goals.
• Educate clients, families, and community caregivers on care plans to maximize independence.
• Conduct evaluations, treatments, and discharge planning using outpatient EMR documentation.

🎯 Requirements

• A current North Carolina Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an OT program accredited by AMA and AOTA (transcript or diploma).
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
