Perform outpatient therapy evaluations to plan care for adults in senior living or group homes.
Use evidence-based methods to maximize functional independence.
Collaborate with home health partners to transition care to outpatient services.
Educate clients, families, and caregivers on care plans to maximize independence.
Conduct evaluations, treatments, and discharge planning using the EMR.
Provide client-centered care in home health settings and communities.

🎯 Requirements

Current New Hampshire OT license.
Prior home care experience preferred but not required.
Minimum one year OT experience.
Graduation from AMA/AOTA-accredited OT program.
Ability to work independently and manage time.
Strong interpersonal/organizational skills; problem solving.
Solid computer skills; EMR experience.
