📋 Description

• Perform outpatient therapy evaluations for seniors in designated living communities.
• Use evidence-based approaches to maximize functional independence.
• Collaborate with home health partners to transition care to outpatient services.
• Educate clients, families, and caregivers on care plans to achieve goals.
• Document and discharge planning using the outpatient EMR.

🎯 Requirements

• A VA Occupational Therapist license.
• Home care experience preferred but not required.
• Minimum of one year OT experience.
• Graduate from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.
