Make outpatient visits to clients in designated senior living communities.
Perform mobility/ROM, transfer status, gait, strength, balance tests.
Develop and implement individualized care plans with manual therapy, gait, balance.
Continually assess and revise PT plan of care and participate in multidisciplinary meetings.
Educate clients, families, and staff on therapy services to promote independence.
Document observations and interventions using a touch-pad tablet.

🎯 Requirements

Massachusetts Physical Therapist license
Geriatrics or senior living experience preferred
Outpatient/home health PT experience
Mobility, gait, balance assessments expertise
Ability to develop and implement care plans
Strong documentation skills on a touch-pad tablet
