Continually assess and revise PT plan; participate in multidisciplinary care meetings.
Educate clients and families on therapy services to promote health and independence.
Document observations and interventions using a touchpad/tablet.

🎯 Requirements

Active Massachusetts Physical Therapist license.
Interest in geriatric outpatient/home health services.
Strong communication and teamwork skills.
Proficiency documenting care plans and progress.
