  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological, and motor aspects; develop intervention plans.
  • Evaluate home environment for hazards and barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Continually assess and revise the OT care plan with nursing.
  • Educate patients and families in rehab and safe living.

🎯 Requirements

  • A current VA Occupational Therapist license.
  • Prior home care experience preferred, but not required.
  • A minimum of one year of recent OT work experience.
  • Graduate from an OT program accredited by AMA/AOTA.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
