Make home visits to clients in designated geographic territories.
Conduct evaluations of functional status including ADL/IADL.
Develop and implement individualized intervention plans with the multidisciplinary team.
Assess home environments for hazards and needed adaptations.
Educate patients and families to promote safety and independence.
Document observations and care using a touch-pad EMR.

🎯 Requirements

Current VA Occupational Therapist license.
Minimum 1 year OT experience.
AMA/AOTA-accredited OT degree program completion.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Proficient with computers; EMR experience preferred.
