Make home visits to clients in designated territories.
Evaluate patients' functional status, endurance, ADL/IADL.
Assess cognitive, psychological, and motor performance; plan interventions.
Identify equipment needs and environmental adaptations.
Educate patients and families on rehabilitative care for safety and independence.
Document observations and evaluations in EMR with touch pad tablet.

🎯 Requirements

VA Occupational Therapist license.
Minimum 1 year OT experience; home care preferred but not required.
Graduation from an accredited OT program.
Ability to work independently and manage time.
Strong interpersonal, organizational and problem solving skills.
Solid computer skills; EMR experience.
