Case Management Nurse

📋 Description

• Educate members to improve health outcomes.
• Assist with transitions between care settings.
• Participate in process improvement and pilot programs.
• Coordinate with support teams for quality care.
• Report to Associate Director, Clinical.
• Occasional travel for team meetings and events.

🎯 Requirements

• Active, unrestricted RN license in the US or compact multistate license.
• Ability to obtain additional state licenses.
• 2+ years clinical experience in payer, hospital, outpatient or community-based care management.
• 1+ years in care coordination and navigation.
• CCM Certification is a plus.
• Spanish/Creole language skills are a plus.
