Perform preventive care screenings, medication reconciliation, and treatment as appropriate.
Outreach chronically ill members based on historical and suspected health information.
Engage members less likely to see their PCP.
Cross-train into urgent care and/or virtual primary care.
Interact with members virtually for assessment, diagnosis and treatment.
Ensure compliance with applicable laws and regulations.

🎯 Requirements

DNP, FNP, ENP from an accredited program
Board Certification (AANP or ANCC)
Compact RN license
Bilingual - Spanish fluency (read, write, speak)
Licensed in at least two states: FL, TX, OH, NJ
3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care
