Conduct in-person and virtual patient assessments; optimize therapy.
Order and monitor evidence-based treatment plans; resolve medication-related problems.
Support quality-improvement and REMS initiatives.

🎯 Requirements

Doctor of Pharmacy (PharmD) required.
Licensed pharmacist in Oregon.
PGY2 Oncology Residency.
PGY1 Ambulatory Care Residency + 3+ years oncology pharmacy experience.
3+ years oncology pharmacy experience with CPA (1 year).
Strong communication, organization, and technology skills.
Empathy, integrity, and team-first mindset.
