- Perform home visits to clients in the Northern VA area per care plan.
- On-call duties; respond to calls Mon-Thu 5:00p-8:30a and Fri 5:00p-Mon 8:30a.
- Follow designated care plans and provide RN care per patient needs.
- Administer medications, wound care, treatments, and procedures.
- Monitor client conditions and report changes to Clinical/Client Services Manager.
- Document orders and care notes; coordinate care with team.
🎯 Requirements
- Active VA RN license.
- At least 1 year clinical (medical/surgical) experience.
- Home health experience preferred.
- Graduation from an accredited nursing program.
- Ability to work independently and manage time.
- Strong interpersonal skills; EMR experience preferred.
