📋 Description

  • Make home visits to clients in designated geographic territories.
  • Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, and ADL/IADL performance.
  • Assess cognitive, psychological and motor aspects of performance; develop individualized intervention plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations to improve outcomes.
  • Continually assess and revise the OT care plan with nursing in the multidisciplinary team.

🎯 Requirements

  • A current South Carolina Occupational Therapist license.
  • Minimum one year of recent OT experience; home care preferred but not required.
  • Graduation from an OT program accredited jointly by the AMA and the AOTA.
  • Ability to work independently with effective time management.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
