Occupational Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients' functional status (ADL/IADL) and functional skills.
• Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Update the OT care plan and participate with nursing in the multidisciplinary team.
• Educate patients and families in rehab care to promote safety and independence.
• Document observations and interventions using a touch-screen tablet (EMR).

🎯 Requirements

• Current MD Occupational Therapist license.
• Home care experience preferred, not required.
• Minimum 1 year of OT experience.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
