Occupational Therapist, OT, Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patient functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor performance and plan interventions.
• Evaluate home safety and barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Update care plan and collaborate with nursing.

🎯 Requirements

• A current DE Occupational Therapist license.
• Graduation from an occupational therapy program accredited jointly by the AMA and the AOTA, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
