📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status; ADL/IADL, endurance, vision, and communication.
• Assess cognitive, psychological, and motor aspects; create care plans.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions; collaborate on the care plan.

🎯 Requirements

• Active Delaware OT license.
• Minimum 1 year OT experience.
• OT program accredited by AMA/AOTA.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
