πŸ“‹ Description

  • Make home visits to clients in designated territories.
  • Evaluate patients' functional status, including ADL and IADL performance.
  • Assess cognitive, psychological and motor aspects; develop intervention plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Continually assess and revise the OT care plan with nursing in the multidisciplinary team.
  • Educate patients and families in rehabilitative care and safety.
  • Document observations, interventions, and evaluations in EMR.

🎯 Requirements

  • A current North Carolina Occupational Therapist license.
  • Prior home care experience preferred, but not required.
  • A minimum of one year of recent OT experience.
  • Graduation from an OT program accredited by AMA and AOTA.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem solving skills.
  • Solid computer skills; EMR experience preferred.
