  • Make home visits to clients in designated geographic territories.
  • Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological and motor performance; develop intervention plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Revise the OT care plan; collaborate with nursing in the multidisciplinary team.

🎯 Requirements

  • Current North Carolina OT license.
  • Minimum 1 year OT experience; home care preferred.
  • OT program accredited by AMA and AOTA.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
