  • Make home visits to clients in designated geographic territories.
  • Evaluate functional status: muscle strength, endurance, coordination, communication, ADL/IADL.
  • Assess cognitive, psychological and motor performance; develop individualized intervention plans.
  • Evaluate home environment for hazards or barriers to independent living.
  • Identify equipment needs and environmental adaptations to improve outcomes.
  • Continually assess and revise the OT care plan; coordinate with nursing as appropriate.

🎯 Requirements

  • A current North Carolina Occupational Therapist license.
  • Prior home care experience preferred, but not required
  • A minimum of one year of recent work experience as an Occupational Therapist.
  • Graduate from AMA/AOTA-accredited OT program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
