Occupational Therapist, OT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle, endurance, vision, communication, ADL/IADL.
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Accurately document observations, interventions, and evaluations using EMR tablet.

🎯 Requirements

• A current DE Occupational Therapist license.
• One year of OT experience minimum.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
