📋 Description

  • Make home visits to clients in designated geographic territories.
  • Evaluate patients’ functional status, including ADL/IADL.
  • Assess cognitive, psychological and motor aspects; develop intervention plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Revise care plan; collaborate with nursing on the multidisciplinary plan.
  • Educate patients and families in rehabilitative care to promote safety and independence.
  • Document care observations and evaluations on a touchpad tablet.

🎯 Requirements

  • Current NC OT license.
  • Minimum 1 year OT experience; home care experience preferred.
  • ACOTE-accredited OT program.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
