Occupational Therapist, OT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients' functional status including ADL/IADL, endurance, and coordination.
• Assess cognitive, psychological, and motor performance; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Review and revise the OT care plan with the multidisciplinary team.

🎯 Requirements

• Current Delaware Occupational Therapist license.
• One year of OT experience; home care preferred.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
