Occupational Therapist, OT - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status: muscle function, endurance, vision, ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and revise the OT care plan; collaborate with nursing.
• Educate patients and families on rehab for safety and independence.
• Document observations and care using a touch pad tablet.

🎯 Requirements

• Current Florida OT license.
• Minimum 1 year OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
