Occupational Therapist, OT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform functional status evaluations (ADL/IADL, endurance, strength).
• Assess cognitive, psychological, and motor performance; develop treatment plans.
• Evaluate home environment for hazards and adaptation needs.
• Collaborate with nursing in multidisciplinary plan; revise OT care plan.
• Document observations and interventions in EMR via touch tablet.

🎯 Requirements

• A current PA Occupational Therapist license.
• A minimum of one year of recent OT experience.
• Graduation from an OT program accredited by AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
