Occupational Therapist, OT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status including ADL/IADL, endurance, and coordination.
• Assess cognitive, psychological, and motor performance; plan care.
• Evaluate home environment for hazards; identify adaptations.
• Document observations and interventions on a touch pad tablet.
• Educate patients and families; participate in multidisciplinary plan.

🎯 Requirements

• Current OT license (OT license required).
• One year of OT experience preferred; home care valued.
• Graduation from accredited OT program (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
