📋 Description

  • Make home visits to clients in designated geographic territories.
  • Perform thorough evaluations of patients’ functional status, including ADL and IADL.
  • Assess cognitive, psychological and motor aspects; develop intervention plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations for better outcomes.
  • Educate patients and families; document observations in EMR.

🎯 Requirements

  • Current North Carolina OT license.
  • Prior home care experience preferred, but not required.
  • Minimum 1 year OT experience.
  • Graduation from AMA/AOTA-accredited OT program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational and problem-solving skills.
