📋 Description

  • Make home visits to clients in designated geographic territories.
  • Evaluate patients’ functional status, including muscle, endurance, vision, and ADL/IADL performance.
  • Assess cognitive, psychological and motor performance; develop individualized intervention plans.
  • Evaluate home environments for hazards and independence barriers.
  • Identify equipment needs and environmental adaptations to enhance outcomes.
  • Continually assess and revise OT care plans; collaborate with nursing in multidisciplinary care.

🎯 Requirements

  • A current North Carolina Occupational Therapist license.
  • Prior home care experience preferred, but not required.
  • A minimum of one year of recent work experience as an Occupational Therapist.
  • Graduation from an accredited OT program (AMA/AOTA) as indicated by transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills, organizational skills, and problem solving skills.
