  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological and motor performance; plan outcomes.
  • Assess home hazards and barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Revise OT care plans; collaborate with nursing in the multidisciplinary team.
  • Educate patients and families on rehab and safety.

🎯 Requirements

  • North Carolina OT license.
  • Prior home care experience preferred, not required.
  • At least 1 year OT experience.
  • Graduate of an accredited OT program (AMA/AOTA).
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
