Occupational Therapist - Home Visits OT

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status (muscle, endurance, vision, ADL/IADL).
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Continually assess and revise the OT care plan with the multidisciplinary team.

🎯 Requirements

• Massachusetts Occupational Therapist license.
• Minimum of one year OT experience.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
