  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological, and motor performance; plan interventions.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Continuously assess and revise the OT care plan; coordinate with nursing.
  • Educate patients and families in rehabilitative care for independence.

🎯 Requirements

  • Current Maryland OT license.
  • Home care experience preferred but not required.
  • Minimum 1 year OT experience.
  • OT program accreditation by AMA and AOTA; transcript/diploma.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational, problem-solving skills.
  • Solid computer skills; EMR experience preferred.
