Occupational Therapist- Home Health Visits

📋 Description

• Visit clients at home in designated territories.
• Evaluate functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor factors; plan interventions.
• Evaluate home environment for hazards or barriers.
• Identify equipment needs and environmental adaptations.
• Update care plan and coordinate with nursing in the team.
• Educate patients and families on rehabilitative care for independence.
• Document observations and interventions in EMR via tablet.

🎯 Requirements

• Pennsylvania OT license current.
• Prior home care experience preferred, but not required.
• Minimum one year OT experience.
• Graduation from AMA/AOTA-accredited OT program with transcript or diploma.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• EMR experience preferred.
