- Make home visits to clients in designated geographic territories.
- Evaluate functional status including muscle function, endurance, vision, written and verbal communication skills, and ADL and IADL performance.
- Assess cognitive, psychological and motor performance; develop individualized intervention plans to achieve favorable outcomes.
- Evaluate home environment for hazards or barriers to independence.
- Identify equipment needs and environmental adaptations for better outcomes.
- Continually assess and revise the OT care plan; participate with nursing in the multidisciplinary care plan.
- Educate patients, family members, or other representatives in rehabilitative care to promote health, safety and independence.
- Accurately document observations, interventions, and evaluations using a touch pad EMR.
π― Requirements
- A current Maryland OT license.
- Prior home care experience preferred, but not required.
- Minimum one year of recent Occupational Therapist experience.
- Graduation from an AMA/AOTA-accredited OT program.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.
