Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status including muscle function, endurance, vision, written and verbal communication skills, and ADL and IADL performance.
• Assess cognitive, psychological and motor performance; develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Continually assess and revise the OT care plan; participate with nursing in the multidisciplinary care plan.
• Educate patients, family members, or other representatives in rehabilitative care to promote health, safety and independence.
• Accurately document observations, interventions, and evaluations using a touch pad EMR.

🎯 Requirements

• A current Maryland OT license.
• Prior home care experience preferred, but not required.
• Minimum one year of recent Occupational Therapist experience.
• Graduation from an AMA/AOTA-accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
