Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients' functional status (muscle function, endurance, vision).
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations, interventions, and evaluations using EMR.

🎯 Requirements

• A current STATE Occupational Therapist license.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by AMA and AOTA, as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
