Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of functional status including ADL/IADL and motor skills.
• Assess cognitive, psychological and motor aspects; develop individualized intervention plans.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Continuously assess and revise the OT care plan with nursing.
• Educate patients and family in rehabilitative care for safety and independence.
• Document observations and interventions using a touch-pad tablet.

🎯 Requirements

• Maryland OT license (active).
• Minimum 1 year OT experience.
• Graduate from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
