Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, ADL/IADL, coordination.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations; revise care plan.
• Educate patients and families and document care using EMR on a tablet.

🎯 Requirements

• Current Maryland Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum one year OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills; EMR experience preferred.
