📋 Description

  • Make home visits to clients in designated geographic territories.
  • Evaluate patients’ functional status, including ADL/IADL performance.
  • Assess cognitive, psychological and motor aspects; plan interventions.
  • Evaluate home environment for hazards to independence.
  • Identify equipment needs and environmental adaptations.
  • Continuously assess and revise the OT care plan with nursing.
  • Educate patients and families in rehabilitative care for independence.
  • Document observations, interventions and evaluations in EMR.

🎯 Requirements

  • Maryland Occupational Therapist license.
  • Home care experience preferred but not required.
  • Minimum one year of OT experience.
  • Graduation from AMA/AOTA-accredited OT program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
