Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status, including ADL/IADL performance.
• Assess cognitive, psychological, and motor function; plan.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and revise the OT care plan; coordinate with nursing.
• Educate patients and families in rehabilitative care to promote health.
• Document observations and interventions using a touchpad tablet.

🎯 Requirements

• Maryland OT license.
• Prior home care experience preferred, but not required.
• Minimum of one year of OT experience.
• Graduation from an accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
