Occupational Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients' functional status including ADL and IADL.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using EMR on a touchpad tablet.

🎯 Requirements

• Current Pennsylvania Occupational Therapist license.
• Graduation from an OT program accredited jointly by AMA and AOTA (transcript/diploma).
• At least one year of recent OT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
