Occupational Therapist, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: ADL/IADL, endurance, vision, coordination.
• Assess cognitive, psychosocial, and motor aspects; develop intervention plans.
• Evaluate home hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Educate patients and families on rehab, safety, and independence.

🎯 Requirements

• Active SC Occupational Therapist license.
• At least 1 year of OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
