Make home visits to clients in designated territories.
Evaluate patients’ functional status and ADL/IADL performance.
Assess cognitive, psychological, and motor abilities; create care plans.
Evaluate home environment for hazards and barriers to independence.
Identify equipment needs and environmental adaptations for better outcomes.
Continually assess and revise OT care plans; collaborate with nursing as needed.

🎯 Requirements

Current Florida OT license.
Minimum of one year OT experience.
Graduation from an AMA/AOTA-accredited OT program.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
