  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological and motor aspects; develop individualized plans.
  • Evaluate home environment for hazards to independence.
  • Identify equipment needs and environmental adaptations.
  • Assess and revise OT care plan; collaborate with nursing.
  • Educate patients and families on rehabilitative care for safety.
  • Document observations and interventions using a touch pad tablet.

🎯 Requirements

  • Maryland Occupational Therapist license.
  • One year OT experience minimum.
  • Graduation from an accredited OT program.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
  • Prior home care experience preferred.
