Occupational Therapist - Home Health, Senior Living Facilities

📋 Description

• Perform outpatient therapy evaluations to develop care plans in senior living communities or group homes.
• Operate at top of licensure using current research-based care approaches to maximize functional independence.
• Collaborate with home health therapy partners to transition care to outpatient services for client-centered goals.
• Educate clients, families, and caregivers on care plans to maximize independence.
• Perform evaluations, treatments and discharge planning using outpatient EMR to document care.

🎯 Requirements

• Current NJ Occupational Therapist license.
• Minimum 1 year experience as an Occupational Therapist.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
