Occupational Therapist- Home Health, Senior Living Facilities

📋 Description

• Perform outpatient therapy evaluations to develop care plans for seniors in living communities.
• Operate at top of licensure with evidence-based approaches to maximize independence.
• Collaborate with home health to transition care to outpatient services aligned with client goals.
• Educate clients, families and caregivers on care plans to maximize independence.
• Conduct evaluations, treatments and discharge planning via outpatient EMR.
• Service area includes Somerset, Middlesex and Monmouth Counties.

🎯 Requirements

• A current NJ Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum one year of OT experience.
• Graduation from an AMA/AOTA accredited OT program.
• Ability to work independently and manage time.
• Solid computer skills; EMR experience preferred.
