📋 Description

• Perform outpatient therapy evaluations to plan care for seniors.
• Use evidence-based approaches to maximize functional independence.
• Coordinate with home health partners to transition to outpatient services.
• Educate clients, families, and caregivers on care plans.
• Conduct evaluations, treatments and discharge planning in outpatient EMR.

🎯 Requirements

• NJ Occupational Therapist license.
• Prior home care experience preferred.
• Minimum of one year OT experience.
• AMA/AOTA-accredited OT program completion.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
