Occupational Therapist- Home Health, Senior Living

📋 Description

• Perform outpatient therapy evaluations to develop care plans for seniors in living communities.
• Operate at top of licensure using evidence-based care to maximize independence.
• Coordinate with home health therapy to transition care to outpatient services for client goals.
• Educate clients, families, and caregivers on care plans to maximize independence.
• Document evaluations, treatments, and discharge planning using outpatient EMR.
• Contribute to a multidisciplinary team to support client-centered outcomes.

🎯 Requirements

• A current NJ Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent experience as an Occupational Therapist.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
